Services Provided

Ivory Bloom Wellness offers IV therapy, vitamin injections, NAD+ therapy, wellness protocols, weight loss optimization, and medical aesthetic services. All services are administered by a licensed Registered Nurse under physician oversight and are subject to medical clearance via a Good Faith Exam prior to treatment.

Services are provided for wellness and aesthetic purposes only and do not constitute emergency medical care. We do not diagnose, treat, or cure medical conditions.

Medical Clearance & Good Faith Exam

All clients are required to complete a Good Faith Exam prior to receiving any treatment. This evaluation is conducted by a licensed medical provider and includes a review of your health history, current medications, contraindications, and treatment goals. A Good Faith Exam is required once yearly or sooner if your health status changes.

We reserve the right to decline service to any client for whom treatment is deemed medically inappropriate.

Appointments & Cancellations

Appointments are required for all services. We ask that you provide at least 24 hours notice if you need to cancel or reschedule your appointment. Late cancellations or no-shows may result in a cancellation fee at our discretion.

We reserve the right to reschedule or cancel appointments when necessary and will make reasonable efforts to notify you in advance.

Payment

Payment is due at the time of service. We accept all major credit cards, cash, FSA, and HSA. All prices are subject to change without notice. Promotional pricing and packages are non-refundable unless otherwise stated.

We do not accept insurance. All services are paid out of pocket.

Prior to receiving any treatment, clients will be asked to review and sign an informed consent form.
